Forms & Permit Applications
For the most up to date state wetland permit application forms please follow the link below to the MA DEP Web Site - Wetlands and Waterways Forms Section:


All state wetlands permits must be submitted with the following forms attached:
1.  Affidavit of Service (Indicating that abutters have been notified)
2.  A copy of the abutter notification form
3.  Sturbridge filing fee worksheet
4.  Finance Department tax sign off

Please note the following:
-Applicants may use Certificates of Mailing to Notify Abutters.
-Applicants do not have to provide a copy of the application materials to the Town Clerk (the Town Clerk will not accept them).
-Upon receiving a Legal Ad from the Conservation Agent the applicant is responsible for getting the legal advertisement to the Southbridge Evening News for publication prior to the noted deadline on the legal ad.
-Applicant is responsible for presenting the Newspaper Tearsheet (proof of publication) and the Certificates of Mailing/Return Receipt cards prior to the public hearing being opened.




PERMIT
DESCRIPTION
Provides basic guidance on submitting an RDA.
(Requests for Determination of Applicability) are used for small projects outside wetland areas where the applicant requests that the Commission determine if the Wetlands Protection Act is applicable to the proposed work activity. Simple projects with proper protective measures, such as hay bales, are typically dispositioned using an RDA. It can also be used to request a ruling from the Commission as to whether a particular site might contain protectable wetlands or other resource areas.
Provides basic guidence on submitting a NOI
Notices of Intent are used for larger projects, such as new home construction, major projects, activities involving wetlands work, such as lake front repairs and driveway crossings through wetland areas. Notices of Intent involve two or more hearings and are more detailed than RDAs.
Provides general guidance on submitting an ANRAD
The ANRAD is process that is used to approve the wetland resource areas on property. Typically, an applicant will file an ANRAD application to have the wetland resource area delineation approved by the Conservation Commission--this is good for large parcels of land. Once the wetland delineation is approved, then the Applicant can design a project to avoid the wetland resource areas. The ANRAD Form also includes the Simplified Review process.
 
A Request for Certificate of Compliance should be submitted to the Conservation Commission once ALL work is complete and all Conditions listed in an issued Order of Conditions are met.
Certification that abutters have been notified in accordance with the regulations.
Form filled out by the applicant and sent via certified mail/certificate of mailing to abutters in accordance with the regulations
This form accounts for the filing fee based on the Sturbridge Wetland Protection Bylaw Regulations
Cerifies that all taxes are paid on the subject property
